Clostridium tetani
Tetanus is an acute infectious disease caused by toxigenic strains of the bacterium Clostridium tetani (C. tetani). C. tetani is a Gram-positive, spore-forming, strictly anaerobic bacterium. Spores are ubiquitous in the environment, especially in warm and humid soils, and can be carried in the intestines tracts and feces of humans and animals. C. tetani enter the body through contaminated skin wounds or tissue damage, including those resulting from unclean deliveries, burns, surgery, and dental extractions, with a high mortality rate. The most important toxin of C. tetani is the highly potent tetanospasmin, which blocks inhibitory neurotransmitters in the central nervous system and causes the muscular rigidity and spasms typical of generalized tetanus.
Vaccines for Clostridium tetani
Tetanus vaccines are produced by the mechanism of inhibition tetanus toxin. The toxigenic strain of C. tetani grows in a liquid medium that favors toxin production, and the toxin produced therefrom is purified and then treated with formaldehyde to produce a toxoid antigen by inactivating the toxin. To increase immunogenicity, Tetanus toxoid (TT) is then formulated with aluminum or calcium salts and administered by intramuscular injection. Tetanus toxoid vaccines are produced in two forms, either as a single form of antigenic vaccine (TT) or in combination with diphtheria toxoid (DT), or mixed with diphtheria and whole cells or cellular pertussis (DTP).
The diphtheria-tetanus vaccine does not contain pertussis and can, therefore, be used in children who are intolerant of pertussis vaccine. Single-antigen tetanus toxoid vaccines containing only tetanus toxoid are usually not given to infants and children. Immunization against tetanus by vaccine alone is not enough, so it is necessary to immunize adults and adolescents by continuously using monovalent tetanus toxin as a booster dose to enhance immunity. Almost tetanus toxoids are combined with small amounts of other vaccines such as diphtheria DT or cell pertussis Tdap.
