Interleukin 8 (IL-8)

Interleukin 8 (IL-8), alternatively known as chemokine (C-X-C motif) ligand 8 (CXCL8), is a chemokine encoded by the CXCL8 gene in human. IL-8 is predominantly produced by macrophages, as well as other cell types such as epithelial cells, endothelial cells, airway smooth muscle cells and tumor cells. Endothelial cells store IL-8 in their storage vesicles, called the Weibel-Palade bodies. IL-8 is a significant mediator of the immune reaction in the innate immune system response, including immune surveillance, inflammation, and angiogenesis, via a chain of biochemical reactions. It also functions as a potent angiogenic factor in tumors by modulating endothelial cell proliferation and migration. There are numerous receptors on the surface membrane, which is capable of binding IL-8, including the G protein-coupled serpentine receptors CXCR1 and CXCR2.

IL-8 and Renal Parenchymal Injury

Multi-inflammatory responses have been found to be involved in ischemia/reperfusion injury of the kidney, through both animal and human studies. The inflammatory reaction caused by an ischemic insult brings on more damage to the tissue and hampers intrarenal blood flow caused by vascular congestion and vasoconstriction, resulting in a vicious cycle. This injury predominantly occurs in endothelial cells of the peritubular capillaries, particularly in the outer medulla, which is marginally oxygenated in ordinary circumstances, Overstimulation and dysfunction of these induce kidney macrophages and probably renal parenchymal cells to produce inflammatory cytokines, such as TNF-α (tumor necrosis factor-α) and IL-1 (interleukin-1). TNF-α and IL-1 accelerate the secretion of the chemoattractant interleukin-8 by renal cortical epithelial cells. And these cytokines precipitate renal parenchymal injury through directly inducing apoptosis in epithelial cells, recruiting neutrophils that secrete reactive oxygen metabolites and proteases, and up-regulating adhesion receptors on endothelial cells and leukocytes.

IL-8 Marker of Sepsis

IL-8 may play a crucial role in the pathophysiology of sepsis via its neutrophil-activating properties. Circulating IL-8 is measured levels in patients with clinical sepsis and the results demonstrate that the expression levels of IL-8 are increased in most patients with sepsis and associated with some important clinical, biochemical, and inflammatory parameters. It is suggested that IL-8 play a role in the pathophysiology of sepsis. IL-8 may be a valid and early predictive marker of neonatal infection. In addition, IL-8 is associated with severity of the infection.
