N-acetyl-β-(D)-glucosaminidase (NAG)

N-acetyl-β-(D)-glucosaminidase (NAG) is a lysosomal enzyme of proximal renal tubular cells. It functions as an acidic hydrolase which hydrolyzes N-acetyl-β-D-glucosidase linkage existed in glycoprotein and lipoprotein. NAG is a macromolecular protein and the molecular weight is about 140,000. It almost distributes in all tissues, including prostate and kidney. Especially in the proximal renal tubular lysosome, it has the highest concentration. The NAG has many isoenzymes, and type A and B isoenzymes are the major form of existence in most tissues. Certainly, there are trace amounts of I1, I2S, and P isozymes in a few tissues. There are three kinds of isoenzymes, NAG-A, NAG-B and NAG-I in kidney tissue.

NAG Marker of Acute Kidney Injury (AKI)

AKI is sudden episode of kidney failure or kidney damage that happens within a few hours or a few days. Decreased kidney blood flow, exposure to substances harmful to the kidney and an inflammatory process in the kidney may lead to AKI occurrence. AKI is a complex clinical syndrome which is difficult to predict and identify in the early period.

Most studies have been shown that urinary NAG may serve as an early detection and prognostic marker in AKI. NAG is a lysosomal enzyme which is major located in the proximal tubular epithelial cell. Due to its large molecular weight, it can not be filtered by glomerular. Normally, NAG activity in the renal tissue, especially in renal tubular epithelial cells is much higher than the ureter and lower urethra. However, when the kidney suffers from damage and injury, NAG will leak into urine, resulting in increased NAG activity in the urine. Hence, urinary NAG activity may reflect renal parenchymal lesions. And an increase in the urinary activity of NAG is a sensitive and reasonably specific measure of AKI. Besides, the urinary NAG activity is increased in chronic nephritis, chronic renal failure, lupus nephritis, nephrotic syndrome, rejection after renal transplantation, toxic nephropathy, epidemic hemorrhagic fever, late cirrhosis and so on.
