CD117

CD117, also known as proto-oncogene c-Kit, Mast/stem cell growth factor receptor (SCFR) or tyrosine-protein kinase kit, is a human receptor tyrosine kinase protein encoded by the KIT gene. CD117 is an important cell surface marker, a cytokine receptor expressed on the surface of hematopoietic stem cells and on other cell types. CD117 can be used to identify certain types of hematopoietic progenitors in the bone marrow. CD117 is a type III receptor tyrosine kinase that binds to stem cell factor, a substance that causes the growth of certain types of cells, also known as "steel factor" or "c-kit ligand". When this receptor binds to the stem cell factor (SCF), it forms a dimer which activates its intrinsic tyrosine kinase activity, which in turn phosphorylates and activates signal transduction molecules that propagate the signal in the cell. Signaling through CD117 is important in cell survival, proliferation, and differentiation. Thus, the altered form of CD117 receptors may be associated with some types of cancers.

CD117 as Marker of Gastrointestinal Cancer

Gastrointestinal stromal tumor (GIST) is the most common gastrointestinal mesenchymal tumor. GIST is derived from smooth muscle pacemaker interstitial cell of Cajal or similar cells. Histopathologists found it hard to tell the difference between the types of molecules decades ago. Subsequently, CD34 and later CD117 were identified as markers to distinguish the various types. Most of GISTs (66%) occur in the stomach and gastric GISTs have a lower malignant potential than tumors found elsewhere in the GI tract.

About 85% of GISTs are associated with abnormal c-KIT pathways. KIT is a gene encoding a transmembrane receptor for a growth factor. CD117, the product of c-KIT is expressed on ICCs and numerous other cells, predominantly melanocytes, mast cells, bone marrow cells, and several other cells. However, in the gut, CD117-positive mass is likely to be GIST produced by ICC cells.

Contrary to the causes of other similar tumors, pathologists can use the immunohistochemical, specific antibody to CD117, when GIST is suspected. 95% of all GISTs are CD117 positive. Other possible markers include DOG-1, CD34, vimentin, and desmin. Other cells showing positive for CD117 are mast cells.
