Introduction of Calcium

Calcium has diverse important physiological functions including muscle contraction, nerve conduction, hormone release, blood coagulation, and some metabolic processes, like bone modeling and remodeling. The balance of precipitated bone calcium and serum ionized calcium is maintained through the circulating parathyroid hormone (PTH), vitamin D, and calcitonin. PTH, secreted by the parathyroid glands, could increase the serum calcium level by mobilizing calcium from bone, decreasing bone resorption, and increasing the renal and intestinal absorption of calcium. While calcitonin, produced by the parafollicular cells of the thyroid gland, deregulates high serum calcium in the opposite.

Normally, the concentration of serum calcium is kept at a stable level. But disorders of calcium metabolism balance happen when the serum calcium concentration exceeds the minimum or maximum limits, which clinically present as hypocalcemia, hypercalcemia, bone disease, kidney disease, arteriosclerotic vascular disease, and even cancers. Therefore, serial measurements of serum calcium, a sensitive and non-invasive biomarker, could be used for the diagnosis of disease and toxicity assessment.

The total serum calcium includes free ionic calcium, complexed calcium, and protein-bound calcium. Above 90% of the protein-bound calcium is linked to albumin with the remaining 10% bound to a variety of globulins. Therefore, there’s a need to consider the adjustment for albumin concentration to deal with abnormal calcium values and reasonable concentration changes individually.

Serum calcium could be determined by various methods, including EDTA titrimetric method, fluorimetric method, colorimetric method, atomic absorption spectrophotometry, and photonic polymer film method. Many of these procedures could be performed on semi-automatic or fully automated biochemistry analyzers, which makes it a routine service at Creative Biolabs.
