Introduction to 8-OHDG

8-hydroxy-2'-deoxyguanosine (8-OHdG) or 8-oxo-7,8-dihydro-2'-deoxyguanosine (8-oxodG) is one of the major forms of free radical-induced oxidative lesions in nuclear and mitochondrial DNA. In other words, 8-OHdG is a product of ROS attacking DNA. The levels of 8-OHdG are increased in both mitochondrial DNA and nuclear DNA with age. Furthermore, the increasing levels of 8-OHdG likely contribute to carcinogenesis by regulation of gene expression and induction of mutations. 8-OHdG has been shown to regulate epigenetic alteration such as CpG islands in a promoter region of a gene which could reduce the expression of the tumor suppressor gene and contribute to carcinogenesis. Currently, 8-OHdG has been widely used as a biomarker for oxidative stress and carcinogenesis. Also, 8-OHdG has a key role during the formation of memory by promoting thousands of rapid demethylations of methylated cytosines in CpG sites.

8-OHDG as A Diagnostic Biomarker for Different Diseases

The biological significance of alterations in 8-OHdG levels has been studied in multiple diseases. Elevated levels of 8-OHdG have been reported to be correlated with clinical outcomes of stroke. Besides, serum 8-OHdG was identified as a diagnostic marker for nonalcoholic steatohepatitis (NASH) when comparing the serum 8-OHdG levels in healthy controls and patients with nonalcoholic fatty liver (NAFL) and NASH. Moreover, increased 8-OHdG levels are closely related to the accumulation of mutations triggering carcinogenesis. Studies have reported that a high 8-OHdG concentration in leukocyte DNA was associated with advanced age and poor prognosis of patients with serous ovarian cancer (SOC) in comparison to controls. Also, when comparing the blood levels of 8-OHdG in women with benign breast tumors, breast cancers, and healthy controls, researchers have found a significant increase of serum levels of 8-OHdG in patients with breast cancer and that 8-OHdG can be used as a potential noninvasive biomarker for early detection of breast cancer.
