Background of Insulin Autoantibody
Insulin is a key peptide hormone produced by beta cells of the pancreatic islets and is considered as the major anabolic hormone of the body. It modulates the metabolism of carbohydrates, fats, and proteins by enhancing carbohydrate absorption, especially the absorption of glucose from blood into liver, fat and skeletal muscle cells. Insulin autoantibodies (IAAs) were first described by Palmer and co-workers in 1983. These autoantibodies appear before insulin treatment and exist in approximately 70% of children and adolescents at the diagnosis of type 1 diabetes (T1D). There is a strong correlation between IAAs level and age, that is, more than 90% of children under 5 years old were found IAAs at the time of diagnosis, which makes them particularly useful for diabetes prediction in young children.

The Role of Anti-insulin in Type 1 Diabetes
T1D is usually diagnosed in children and young adults, formerly known as juvenile diabetes. Once T1D develops, it needs lifelong treatment. After the onset of T1D, the body cannot produce enough insulin, and unless someone takes measures to control hyperglycemia, the blood sugar level is still very high. As early as the 1950s, it was recognized that circulating anti-insulin antibodies (IAs) existed in patients who treated with insulin. Since the use of high purity animal and human insulin preparations, the prevalence and titers of IAs have decreased significantly. At present, the best model for predicting diabetes is based on the combined measurement of islet autoantibodies. IAs are the key components of this combination. The early emergence of IAA has made it an indispensable factor for the development of islet autoimmunity from birth in natural history and major intervention studies. The IAA response intensity reflected by antibody levels changed the risk of diabetes progression. Therefore, IAs are very important as predictors of early T1D and as indicators of continuous subcutaneous insulin infusion or multiple daily insulin injections in T1D patients.
