Overview of Atherosclerosis
Atherosclerosis is a common chronic inflammatory disease that occurs worldwide. Atherosclerosis presents in large and medium-sized arteries and is mainly characterized by diffuse medial intimal fibrosteatosis. Scientists have found that atherosclerosis is strongly associated with the onset of stroke, heart failure, and peripheral vascular disease. Atherosclerosis involves multiple factors, such as age, smoking, metabolism, infectious agents, and genetic factors. Although various factors have different effects on lesion formation, the growth of atherosclerotic lesions is closely related to low-density lipoprotein (LDL) levels and dysfunctional high-density lipoprotein (HDL) levels. Notably, oxidation of LDL plays a central role in the atherosclerotic process and produces a variety of oxidatively modified lipids and proteins that represent determinants of the high immunogenicity of the immune system.
Pathology of Atherosclerosis
Atherosclerosis is a lipid-driven inflammatory disease of the arterial intima, and the balance of anti-inflammatory and pro-inflammatory mechanisms is decisive for the final clinical outcome. The hallmark of chronic atherosclerotic inflammation is the progressive accumulation of lipids and inflammatory cells. Indeed, extracellular modification of infiltrating plasma lipoproteins may be an early event in atherosclerosis. Intimal infiltration and modification of plasma-derived lipoproteins are taken up by macrophages, followed by the formation of lipid-laden foam cells that initiate the formation of atherosclerotic lesions. While foam cells are beneficial in atherosclerotic lesions early in their formation, they can be detrimental in advanced lesions, as foam cells die and release their contents into the extracellular space, thereby worsening the inflammatory state of the plaque and structural stability.
