Salmonella typhi (S. typhi) is a bacterium that infects the intestinal tract and the blood and can cause typhoid fever in humans. The bacterium infection is serious and potentially life-threatening. S. typhi is a facultative anaerobic, gram-negative bacterium that belongs to the family Enterobacteriaceae and the genus Salmonella. S. typhi is spread by the fecal-oral route and the contaminated water and food are the common sources of typhoid fever in endemic areas, so S. typhi is common in many developing countries where sewage and water treatment systems are poor. After exposure to the bacteria, symptoms start an average of 1 to 2 weeks include constipation, high fever, headache, fatigue, loss of appetite, dizziness, cough, and a rash on the trunk.
Live Typhoid Vaccines
There is a live attenuated bacterial vaccine called Ty21a available that protects against typhoid for oral administration only. The vaccine is comprised of live-attenuated Ty2 strain of S. typhi in which multiple genes, including the genes responsible for the production of Vi, have been mutated chemically so as to render it harmless but nevertheless immunogenic. It is in capsule form and is recommended for people 6 years of age and older and takes about 2 weeks after injection for the vaccine to begin protecting against typhoid fever. This typhoid vaccine offers a statistically significant protection for the first seven years and a sufficient quantity of complete lipopolysaccharide is synthesized to evoke a protective immune response. The typhoid vaccine contains a small amount of the typhoid bacteria, enough to stimulate your body to create its own defenses but not enough to cause serious illness.
Subunit Vaccine
Another type of typhoid vaccine is the subunit vaccine which made from the purified Vi capsular polysaccharide from the Ty2 S. typhi strain. The administration of this vaccine is injected either under the skin or into a muscle at least seven days before traveling to the typhoid-affected area and the vaccine is not effective in children under the age of two. A typhoid conjugate vaccine composed of Vi polysaccharide conjugated to diphtheria toxoid (Vi-DT) has been developed which was safe, well-tolerated and immunogenic in participants aged 2-45 years. Besides, a novel conjugate form of the vaccine has enhanced efficacy which is comprised of Vi bound to a non-toxic recombinant Pseudomonas aeruginosa exotoxin. The vaccine is indicated for active immunization against typhoid fever caused by S. typhi either in adults or children 2 years of age or older.
Combination Vaccine
The combination vaccine includes hepatitis A and typhoid which can protect against the infectious diseases hepatitis A and typhoid. It is a combination product of inactivated hepatitis A virus and Vi polysaccharide of S. typhi bacteria.
