Background of Anti-5-HT Antibody

5-HT, also known as serotonin, plays an important regulatory role in all kinds of immune responses mediated by T cells, B cells, and macrophages through direct and indirect ways. 5-HT is usually considered as a typical neurotransmitter. It plays a regulatory role in the immune system. 95% of 5-HT in peripheral circulation are synthesized by pheochromocytes in the gastrointestinal tract and then released to blood. 5-HT participates in the autoimmune reaction by mediating the activation of T and B cells in several pathways.

Autoantibodies to 5-HT (anti-5-HT-Abs) are present in the serum of patients with psychiatric disorders. The occurrence of anti-5-HT-Abs could indicate autoantibodies to the receptor, which in turn could result in a disturbance of this receptor. Antibody reactivity is also associated with the autoimmune process of the central nervous system (CNS).

The Role of Anti-5-HT in Alzheimer's Disease

AD is a progressive neurodegenerative disorder manifested by a progressive decline in cognitive functions. The most promising AD biomarkers in platelets are 5-HT and lipids. 5-HT plays an important role in cognition, at the same time, a combination of disturbances in cholinergic and serotonergic functions may play a role in cognitive impairment in AD. It has been reported that platelets’ dense granules are the major storage site for 5-HT in the blood, similar to its vesicular storage in neurons, triggered the idea of using platelets as an experimental system in AD. AD patients always show significantly reduced platelet 5-HT levels; furthermore, lower 5-HT content in patients with Subjective Cognitive Impairment was associated with a higher AD-cerebrospinal fluid (CSF) biomarker burden. Thus, 5-HT seems to be a good candidate biomarker for early AD.
