Background of Anti-Ribonucleoprotein (RNP)

Anti-RNP autoantibodies directly target the U1 particle of an extractable nuclear antigen (ENA) called ribonucleoprotein, which consists of a complex set of proteins and a small nuclear RNA called U1. Ribonucleoprotein is a part of the spliceosome and also includes Smith (Sm) antigen. Sm antigen is the target of the anti-Smith antibody and is often positive in SLE. Normally, anti-RNP autoantibodies are a part of the normal immune system, which can resist against pathogens. However, an elevated value of anti-RNP autoantibodies is associated with various rheumatologic diseases including mixed connective tissue disease (MCTD), systemic lupus erythematosus (SLE), scleroderma, and rheumatoid arthritis (RA). The anti-RNP assays usually use ELISA for the quantitative measurement of immunoglobulin G (IgG) class autoantibodies.

The Role of Anti-RNP Autoantibodies in SLE

Anti-RNP autoantibodies are associated with MCTD, a syndrome characterized by features of SLE, inflammatory muscle disease, and scleroderma. Anti-RNP autoantibody levels are increased in 100% of patients with MCTD and 38%-44% of patients with SLE. Compared with healthy controls, the sensitivity of anti-RNP autoantibodies for the diagnosis of SLE ranged from 3%-46% and the specificity ranged from 98%-100%. While compared to other disease controls, the anti-RNP autoantibodies sensitivity for the diagnosis of SLE ranged from 8%-69% and the specificity ranged from 25%-99%.
