Background of Anti-Transglutaminase 2 Antibodies
Anti-transglutaminase antibodies (ATA) are a kind of autoantibodies against the transglutaminase protein. Antibodies play an important role in the immune system by detecting cells and substances, including foreign antigens (for example, viruses) and substances produced by the body (for example, cancer cells). If antibodies direct against these self-substances, they are called autoantibodies. Sometimes autoantibodies can mistakenly direct against healthy portions of the organism, leading to autoimmune diseases. Currently, ATA can be classified according to 2 different schemes: transglutaminase isoform and immunoglobulin reactivity subclass (IgA, IgG) toward transglutaminases.
The Role of ATG2A in NAA Associated Celiac Disease
Celiac disease is an inflammatory disorder, commonly caused by an incorrect immune response to ingested wheat gluten (consist of gliadin and glutenin) and related cereal proteins in genetically predisposed individuals, leading to villous atrophy, crypt hyperplasia and lymphocytic infiltration in the small intestine. Anti-TG2 antibodies, the specific marker of celiac disease, are deposited locally in the intestine primarily but they have also been found in liver, muscle and lymph nodes. Currently, the anti-TG2 antibody test is an important tool in the diagnosis of celiac disease. The test for anti-TG2/endomysial antibodies is based on the patient serum upon suspicion of celiac disease. If the patient is anti-TG2/endomysial IgA antibodies positive, or IgG antibody is positive but IgA is deficient, or in less common cases of negative serology but high clinical suspicion, then it is essential to perform an intestinal biopsy. Positive identification of the characteristic histologic abnormalities results in a presumptive diagnosis of celiac disease and institution of gluten-free diet. Clinical improvement of the diet would yield a definitive diagnosis.
