Background of Anti-transglutaminase 3 Antibodies

Transglutaminase 3 (TG3) is a soluble enzyme expressed predominantly in differentiating keratinocytes, corneocytes and hair follicles. It belongs to the transglutaminase family, which shows conservation especially of certain enzymatically relevant domains. Recently, TG3 is demonstrated to be an autoantigen in celiac disease, a gluten-dependent autoimmune disorder. Anti-tissue transglutaminase antibodies (anti-tTGs) are a kind of autoantibodies against the transglutaminase protein and show great potential as the diagnostic markers of coeliac disease. Currently, anti-tTGs can be classified according to 2 different schemes: transglutaminase isoform and immunoglobulin reactivity subclass (IgA, IgG) toward transglutaminases.

The Role of Anti-TG3 in Celiac Disease

Celiac disease is an inflammatory disorder, commonly caused by an incorrect immune response to ingested wheat gluten (consist of gliadin and glutenin) and related cereal proteins in genetically predisposed individuals, leading to villous atrophy, crypt hyperplasia and lymphocytic infiltration in the small intestine. TG3 is an enzyme expressed in the epidermis and it is closely associated with, but not identical to TG2. The vast majority of celiac disease patients present circulating and intestinal autoantibodies against TG2 at the time of the diagnosis. In addition, TG3 autoantibodies have been identified in the serum of not only the untreated celiac disease but also of Dermatitis herpetiformis (DH) patients. Recently, a study has elucidated the significance of TG3 autoantibodies in celiac disease. Results showed that TG3 autoantibodies correlate with the age at diagnosis, the severity of clinical symptoms or histological inflammation or damage in celiac disease. In addition, gluten-dependency of TG3 autoantibodies can be evaluated with serial serum samples of celiac disease patients adhering to a gluten-free diet (GFD) treatment.
