Cluster of Differentiation 163 (CD163)
Cluster of differentiation 163 (CD163) is a protein with a molecular weight of 130 kDa encoded by CD163 gene. It is the high-affinity scavenger receptor for the hemoglobin-haptoglobin complex. CD163 protein is consist of a 1048 amino acid extracellular domain, a single transmembrane segment, and a cytoplasmic tail with several splice variants. CD163 functions as an innate immune sensor for gram-positive and gram-negative bacteria. The extracellular domain of CD163, commonly denoted soluble CD163, is released from activated monocytes and macrophages during inflammation and exists in plasma and cerebrospinal fluid. The CD163-expressing macrophages rapidly phagocytose the Hb-Hp complex and degrade it via the hemoglobin-recycling-pathway, in which heme oxygenase-1 (HO-1) produces anti-inflammatory metabolites. Thus CD163 indirectly triggers the anti-inflammatory response.

The Role of CD163 in Sepsis
Soluble CD163 has been shown to play a role in inflammatory as well as infectious diseases and it seems to be an early indicator of the susceptibility to sepsis. Sepsis is the systemic inflammatory response to infection. It is a life-threatening condition that arises when the body's response to infection causes injury to its own tissues and organs. The host inflammatory response in sepsis is complex, involving an interplay between pro- and anti-inflammatory mediators, both of which have divergent effects on pathogen control, systemic inflammation, and susceptibility to recurrent infection. It is reported that 12 h after admission CD163 levels were significantly elevated in patients that developed sepsis during their course of stay. In vitro analyses have shown that soluble CD163 interacts with T-lymphocytes and actively inhibits lymphocyte proliferation. This suggests that CD163 leads to depression of the immune capability when lowered as for instance after uptake into lymphocytes.
