Pentraxin 3 in Sepsis
Pentraxin 3 (PTX3) belongs to the long pentraxin subfamily within the family of pentraxins. This superfamily has cyclic multimeric structure. As a soluble pattern recognition molecule, PTX3 functions in the innate immune system. Innate immunity is a potentially life-threatening inflammatory response to infection that affords the infected host protection against sepsis. In severe sepsis, accumulation of extracellular histones in the blood results in damage to endothelial cells and causes organ failure. Both in vitro and in vivo, PTX3 protects endothelial cells by causing histones to aggregate against histone-mediated cytotoxicity toward endothelial cells.

The Role of Pentraxin 3 in Sepsis
As an acute-phase protein, PTX3 is involved in inflammatory and infectious processes. It has been assessed in studies that the diagnostic and prognostic value of PTX3 in patients with sepsis or septic shock in a medical intensive care unit (ICU). Because of cytotoxic effect on endothelial cells, extracellular histones are considered to be the main cause of septic death, as well as a potential therapeutic target. Based on polyvalent interactions and disorder in the secondary structure, PTX3 interacts with histones to form coaggregates. Additionally, in mouse models of sepsis, the intraperitoneal administration of PTX3 enables the reduction of mortality. It has been suggested that the host protective effects of PTX3 in sepsis are a result of its coaggregation with histones. Trough directed at protecting cells from the toxic effects of extracellular histones, this long pentraxin–specific effect brings a potential basis for the treatment of sepsis.
