Introduction of N/OFQ
Nociceptin/orphanin FQ (N/OFQ) is a 17 amino acid opioid-like peptide with structural similarity to the endogenous k (or KOP)-opioid dynorphin A. It is produced and released by neurones and other tissues from a 30 amino acid precursor ppN/OFQ, and initiates its function to act on numerous brain activities such as pain sensation and fear learning. Studies have shown that the mRNA of N/OFQ precursor (ppN/OFQ) along with its specific receptor nociceptin (NOP) are found in monocytes, lymphocytes, and polymorphonuclear cells, which indicated that the nociceptin system is involved in the inflammatory responses of numerous pathological processes. Moreover, activation of the NOP receptor is known to modulate inflammatory responses, such as mast-cell degranulation, neutrophil rolling, vasodilation, increased vascular permeability, adhesion molecule regulation and leucocyte recruitment.

N/OFQ Marker of Sepsis
In the past years, several studies have been conducted to investigate the role of the nociceptin system in inflammation and sepsis. It has been reported that N/OFQ is produced by immunocytes and acts as an immunomodulator, such as induction of chemotaxis and proliferation of immune cells. And data have shown that N/OFQ has a role in the chemoattraction of leucocytes by mast cells. What's more, mast cells release histamine, which is also increased in sepsis and increases vascular permeability and upregulates adhesion molecules which also show upregulation in sepsis. Most importantly, studies have found that the higher plasma concentration of N/OFQ is associated with higher mortality in patients with sepsis. In this case, N/OFQ can be used as a specific marker for the diagnosis of sepsis.
