Endothelin-1 (ET-1)

Endothelin 1 (ET-1) is a potent vasoconstrictor that in humans is encoded by the EDN1 gene. It is produced by vascular endothelial cells. The protein is proteolytically processed to release a secretory peptide called endothelin-1. Endothelin 1 is one of the three isoforms of human endothelin. Preproendothelin is a precursor of peptide ET-1. Endothelial cells convert pre endothelin to pro-endothelin, which is then converted to mature endothelin released by the cell. Endothelin-1 receptor antagonist is used to treat pulmonary hypertension. Inhibition of these receptors prevents pulmonary vasculature constriction and thus reduces pulmonary vascular resistance.

Endothelin-1 Marker of Sepsis Infections

ET-1 acts as a potent vasoconstrictor and has a mitogenic effect on smooth muscle cells. ET-1 has been shown to be involved in a variety of physiological functions related to the cardiovascular, gastrointestinal, renal, nervous, respiratory and endocrine systems. Due to its short half-life, about 1-7 minutes, and almost complete clearance from the bloodstream by pulmonary passage, CT-proET-1 has been found to stoichiometrically measure ET-1.

ET-1 is associated with myocardial injury and death in patients with septic shock. The concentration of CT-proET-1 is increased in non-survivors of septic shock. CT-proET-1 seems to be very important for long-term mortality. ProADM and proET-1 are particularly attractive biomarkers for a septic shock because they are both endothelium-derived pro-hormones, and their end products have important vasoregulatory opposing effects. It is plausible that the net balance between hormones is of significance for clinical outcomes. Increased concentrations of ADM and ET-1 in patients with systemic inflammatory response syndrome (SIRS) and septic shock have been described and appear to be related to severity and mortality.
