Neopterin

Neopterin is a catabolic product of guanosine triphosphate (GTP), which is a purine nucleotide. Neopterin is a member of the chemical group known as acridine. Neopterin can serve as a marker of cellular immune system activation. The measurement of neopterin concentrations in body fluids such as serum, cerebrospinal fluid or urine provides information on the activation of cellular immune activation in humans under the control of helper T cells type 1. Increased neopterin production may be associated with increased production of reactive oxygen species. The concentration of neopterin also allows estimating the extent of oxidative stress elicited by the immune system. Increased neopterin production is found in HIV, HBV, and HCV; bacterial infections such as Borrelia; parasites such as Plasmodium; autoimmune diseases such as rheumatoid arthritis; malignant tumor diseases.

Neopterin Marker of Sepsis Infections

Neopterin is excreted by monocytes/macrophages mostly in response to interferon gamma stimulation, whereas IL-4 (interleukin - IL), IL-10 and IL-12 inhibit NPT excretion. Neopterin is an early and sensitive marker of cellular response but it lacks specificity. It is considered to be a useful tool in the investigation of clinical cases in which cellular response is crucial. Elevation of NPT often precedes the clinical manifestation of inflammation or seroconversion. The correlation between NPT serum level and the risk of septic complications has been described. Localized infections usually do not increase NPT concentration, whereas the process of persistent inflammation and systemic infection with the activation of cellular response is likely to increase NPT level. Sepsis is the most common cause of NPT elevation. Moreover, higher NPT concentrations correlate with poor prognosis for patients.

Neopterin, which is a non-specific marker of activated T-helper cell 1 dominated immune response, may be a useful marker for monitoring of infectious disease activity during treatment and for more accurate estimation of extent of disease and prognosis. In patients on an intensive care unit with sepsis and septic shock urinary neopterin/creatinine ratios were found to be significantly higher compared to patients with other forms of systemic inflammatory responses syndromes. Serum neopterin levels were higher in non-survivors compared to survivors of sepsis and multiorgan failure scores correlated with neopterin levels.
