Introduction of Renin

Renin is an aspartic protease protein and enzyme which is secreted by the kidneys with a mass of 37 kDa. As an angiotensinogenase, it plays an important role in the body’s renin-angiotensin-aldosterone system (RAAS) by mediating the volume of extracellular fluid (blood plasma, lymph, and interstitial fluid), and arterial vasoconstriction. Generally, the function of rennin is to cause an increase in blood pressure by activating the renin-angiotensin system through cleaving angiotensinogen to yield angiotensin I, which is further converted into angiotensin II. Angiotensin II then constricts blood vessels, increases the secretion of ADH and aldosterone, and stimulates the hypothalamus to activate the thirst reflex. At last, the perfusion pressure of the kidneys will be restored.

Renin Marker of Sepsis

Severe sepsis has a high fatality rate despite powerful technical support and development of advanced treatments. It has been reported that RAAS can be activated in patients with severe sepsis, which may subsequently result in ischemic reperfusion injury or energy metabolic abnormality. Also, activation of the RAAS is part of an intrinsic neuroendocrine response to sepsis-associated circulatory failure, which occurs along with activation of the hypothalamic-pituitary-adrenal axis and the sympathetic nervous system, and release of endothelin and vasopressin. In addition, a previous study has demonstrated that following the occurrence of sepsis, the expression of the angiotensin receptor (ATR) was downregulated. Under this condition, the level of renin can be used as a specific marker for the detection of sepsis.
